The Board issues licenses to practice veterinary medicine in the state of Arizona and certifies veterinary technicians. Licenses and certificates are renewed every two years to remain active, with an expiration date of December 31st of every even-numbered year. All the forms you may need can be found below or via the "Forms" tab.

ATTENTION POTENTIAL EMPLOYERS: New graduates who have not passed the NAVLE are NOT eligible for provisional licenses and may NOT provide services required of a veterinarian! New grads are not allowed to "work under" another veterinarian's license and as graduates, they are no longer covered by provisions in the AZ Veterinary Practice Act related to "veterinary students." These new grads who have not passed the NAVLE and have not been licensed may shadow licensed veterinarians and provide the same services that a veterinary technician/assistant would be allowed to do, assuming they are properly trained to do so.
