Where do I get an application? Does it have to be notarized? May I email my premises application to the Board?

The premises license application can be found under the FORMS tab in the Premises section or one can be mailed to you. The completed, notarizedĀ application cannot be emailed. The original application must be submitted to the Board office along with the required fee.
