I need to report an address change. May I give it to you over the phone or does it need to be in writing?
Statutorily, all changes, whether home or employment, are required to be provided in writing within 20 days of the change. Please use the Change of Address/Employment/Name Form, which can be sent by regular mail, faxed, or emailed. As well, for address and employment changes, you may contact us directly through the website (no form required). Simply go to Contact Us to notify the Board of your change(s).
